Honest to goodness…so much to talk about today! Dr. Souryal is joined by Dr. Brad Bellard and Dr. Scott Blumenthal for a wide-ranging conversation on sports, medicine, and current events. Our first caller has worker’s compensation struggles, then Dr. Scott Blumenthal explains the treatment and recovery process for a herniated disc, and the relation between business and medicine weaves through the whole show. Another caller asks if artificial disc replacement might be a good option for him, a biker’s neck pain travels to his right hand, a 15 year old soccer player had an ACL repair and later a patellar dislocation and her father asks what to do next for this rare injury combination. An avid weightlifter has stingers on his right arm in his sleep and on his bike, but not at the gym and wonders what kind of doctor he should see first. Then we finally get to The Business of Medicine as the doctors discuss some situations where doctors can be reimbursed more for an inferior procedure or equipment from insurance than the option that works well for patients, ending with observations on the efficacy of stem cell procedures.

There are so many great questions for just the right doctors to answer! A restaurant manager asks why MCL and ACL injuries heal and don’t heal. A 53 year old bowler’s knee pop hurts more when he’s sitting down than when he’s walking- does he need an x-ray or an MRI? Spine specialist Dr. Scott Blumenthal explains the common course of back spasms, what to do about them and when to do it, including ice or heat. A cyclist was hit by a motorist and after surgery has back pain and a shorter leg. Would a lift be helpful? Another caller wonders if he should investigate his sternum pop more deeply. An offhand remark about an ACL replacement leads Dr. Souryal to explain the real deal with allografts. A caller has back pain and also groin pain. Are they related? A basketball player has a dislocated toe and wonders about his limited range of motion. A weightlifter has an elbow issue, a runner has Metatarsalgia, and another weightlifter has a lump on his arm after a car accident and needs to know if lifting will damage it more.

The risk/reward equation for the Super Bowl is a hot topic this week. Can Carolina Panthers linebacker Thomas Davis play effectively after getting a metal plate in his right forearm? Tony Romo broke and re-injured his collarbone, would a plate help him? What’s up with the Pro Bowl? Who are the toughest athletes?

If you have sports medicine questions or comments, Dr. Souryal has answers, or at least an opinion! First up, a twisted knee needs to get checked out. Then a jogger with a foot injury and subsequent sensory nerve damage is still having problems over a year later. The Business of Medicine segment at the top of the hour is about morals and money and passion. Dr. Souryal and Dr. Brad Bellard discuss how “bill-ability” sometimes takes precedence over what works for patients. Not on their watch!

A former athlete and current Orthopaedic Trauma PA has pain, limited range of motion and weakness, and even a pop and numbness recently which could be related to a shoulder procedure decades ago. The show ends with an interesting case of a rare shoulder blade injury that could actually be a pain in the neck.

Popular segments are back, starting off with The Business of Medicine on the topic of cringe-worthy medical bills and a reminder for patients to look past the sticker shock to their own premium, deductibles, and copays. A Souryal Story about an ACL patient leads to a discussion about differing rehab protocols between surgeons and physical therapists with sports specialist Dr. Craig Garrison. At the top of the hour, Sports Medicine 101 covers cortisol and cortisone derivatives as anti-inflammatories that can be curative, or prematurely age joints if overused.

The calls start with a Navy veteran thanking the Doc for directing him to get an MRI and the questions keep coming fast and furious by phone, email, and texts. A college runner has pins and needles while running distance, a firefighter has outer knee pain but only when sprinting, and full range of motion has not yet returned for one caller eight weeks after ACL and meniscus surgery. There are several calls about rolled ankles and The Doc explains all about stability issues, even that clunky unfashionable shoes may give the best support.
There are questions about back pain, an arthritic shoulder, a strained quad, another asymmetric quad after a tendon repair, a weird foot sensation, and what a a dip or iron cross may be. You may even find out where to get a tasty bacon margarita.

Spine authority Dr. Scott Blumenthal joins Dr. Souryal to talk of homecomings, tailgating, turf, knees, necks, and backs, too.

Discussions range from the difference between a crick and a pinched nerve to why the pain in a herniated disc may come and go, the difference between mechanical and medical problems, and what an X-ray can and cannot show.

Callers include a a little league coach with a sudden shoulder pain, a hiker who broke her ankle in Hawaii, a runner/biker with a numb foot, a golfer with shoulder trouble, and so many more. An active five year old had a rare PCL injury on a trampoline and her parents wonder if there are new surgical options that may stabilize her knee without requiring her to wear a brace for years. The show ends on a high note with a call about arthritis and a miracle pill.
